How to Book

To provide treatment to your dog, we are required by law to have consent from your Veterinary Surgeon. This also allows us to ensure we have the full details of your dog’s condition and that it is appropriate for them to receive Hydrotherapy and/or Physiotherapy treatment. This is the case for all dogs we see, even those who are not attending due to a medical reason. You can download our consent form and Terms of Business here…

Once your Veterinary Surgeon has completed and sent us the consent form, we will contact you to book your dog’s initial consultation appointment.

As a registered centre we will also require your dog’s Veterinary notes, and will request these from your vet.

Please ensure you have read, and agree to the accompanying Terms of Business.

Insurance Claims

As we are registered with The National Association of Registered Canine Hydrotherapists (NARCH) for Hydrotherapy treatment, and The Association of Chartered Physiotherapists in Animal Therapy (ACPAT) for Physiotherapy treatment, you may be able to claim on your pet insurance policy.

Please check with your insurance provider for details, Millhaven Canine Rehabilitation accepts no responsibility should your insurer refuse to pay for any treatment. Whilst payment for treatment sessions must be made up front, we can assist you with your insurance claim if you require this.

Your insurance company may request an itemised receipt with dates of treatment, we do have a £5 administration fee for this service.
